Medicare comes in four Parts: A, B, C and D. Each part covers a different aspect of your healthcare costs.
Medicare Part A and B coverage comes standard for most Medicare-eligible people, but it doesn’t cover everything. This tool will help you determine if additional coverage may be necessary.
Part A Helps cover hospital, skilled nursing, home health, and hospice expenses.

Part B Helps cover doctor visits, outpatient care, and preventive services.

Part D Helps cover the costs of your prescription drugs.

Part C Also known as Medicare Advantage, these convenient, all-in-one plans cover everything Parts A and B cover, except hospice expenses which is covered by Medicare, plus much of what they don’t. You can even choose from plans that include Part D prescription drug coverage and additional members-only benefits and services.

STEP 1 - Additional Coverage
Original Medicare Parts A and B do not cover the below heathcare expenses. If you need coverage for these items, and you don’t get it through another source such as the Veterans Benefits Administration or a group retirement plan, you’ll want to purchase it through a private health insurance company such as Humana.

STEP 2 - Doctor and Hospital Network
Most Medicare Advantage plans include a network of doctors, hospitals, and clinics that meet certain quality standards and have agreed to contract with a private health insurance company such as Humana to provide services to members at a contracted rate.
There are two primary network types to choose from. Please select the network type that best describes your needs.
HMOHealth Maintenace Organization
These plans are typically less expensive than PPO plans. Many Humana HMO plans have low or $0 premiums and deductibles. However, there are some additional requirements for care that allow these plans to cost less.

• Their coverage only extends to in-network doctors, hospitals, specialists, and clinics – with few exceptions. If you choose to get treatment from an out-of-network provider, you will be responsible for 100% of the cost. However, Humana has a large network, so check with a licensed Humana sales agent to see if your preferred healthcare providers are in-network.

• Most HMO plans require you to select a primary care physician. This doctor oversees your medical needs and refers you to specialists, if necessary. Some HMO plans require you to have a referral from your primary care physician before treatment from a specialist will be covered. In some areas, Humana offers HMO plans that waive both requirements completely.

PPOPreferred Provider Organization
PPO plans typically cost more than HMOs but they come with more flexibility. Most PPO plans require you to pay a monthly premium and have a higher deductible for medical care.

• Unlike some HMOs, you generally are not required to select a primary care physician and you won’t need a referral for specialist visits.

• In most cases, you’ll be covered by both in-network and out-of-network doctors and hospitals. However, your out-of-pocket costs will be lower if you get care from in-network providers.

STEP 3 - Predictable Costs and Potential Savings
Many Humana Medicare Advantage plans include features designed to help make your healthcare expenses more predictable and provide opportunities for additional savings.

Maximum out-of-pocket limit Out-of-pocket expenses are the costs you’re required to pay for medical care, prescriptions, and other healthcare services. These include coinsurance, copayments, and deductibles. Medicare Advantage plans feature a maximum out-of-pocket yearly limit that helps you budget for your medical care more effectively. Once you reach your maximum out-of-pocket costs for Part A and Part B covered services, your plan pays 100% of the remaining costs for most covered services for the rest of the plan year.

Dual-eligible Plans "Dual-eligible" is the term used to describe people who are enrolled in both Medicare (Part A and/or Part B) and Medicaid (full Medicaid benefits and/or assistance with Medicare premiums). These individuals are eligible for help paying their premiums, deductibles, coinsurance, and copayments. In some areas, Humana may have plans that are specifically designed for Dual-eligible consumers. These Dual-eligible Special Needs plans coordinate all the benefits of Medicare and Medicaid, plus include additional coverage and extra benefits all in one easy-to-manage plan. They may also reduce payments for premiums, deductibles, and prescription drugs.
