Do you meet one or more of the following requirements?
• You are age 65 or older.
• You have received Social Security disability or a disability pension from the Railroad Retirement Board for at least two consecutive years.
• You have Lou Gehrig’s disease or End-Stage Renal Disease (permanent kidney failure requiring dialysis or kidney transplant).
